If you prefer to make your donation by mail or if you wish to hand-deliver it to the Foundation office, you can download, print and fill out the appropriate donation form below. Our address is:

For phone or in-person donations, the Foundation office is open Monday through Friday between the hours of 8:30am and 4:30pm.

Donate your car, truck, RV, boat or other vehicle to the Les Turner ALS Foundation. The Foundation accepts donations of vehicles in any condition that are located in the Chicago metropolitan area. We take care of the arrangements and towing, saving you the hassle of selling your vehicle, while you take advantage of the tax deduction!
To make a vehicle donation, complete our information form and return it to the Foundation office with a copy of the title (front only). North Shore Towing, the Foundation's contracted vehicle donation program agent, will contact you to arrange a time and place to pick up the donated vehicle. The Foundation will provide the documentation needed for you to obtain the proper tax deduction.

Every year the Les Turner ALS Foundation receives meaningful gifts from individuals who have made provisions in their will, trust or other estate plan.
You can provide under your will or living trust agreement not only for your loved ones, but for organizations like the Les Turner ALS Foundation, which have made a difference in your life. When making or revising your will or trust you should obtain the assistance of an attorney. Your attorney will tell you how you can:
• Give a specific dollar amount
• Give a share or entire residue of your estate
• Give personal property
• Give real estate
If you provide for the Les Turner ALS Foundation through your estate plan, please let us know so we may acknowledge and thank you. There may be no better way to celebrate the life and memory of a loved one than through a gift in support of their dream, whether it is finding a cure for ALS or providing compassionate support services to those currently battling the disease.

Workplace giving is an easy and effective way to give to the Les Turner ALS Foundation. Whether you work for a private company or a public organization, you can use the ease and convenience of a payroll deduction to make your gift or have your employer match your charitable contributions.
Matching Gifts
Did you know your contribution could be doubled or tripled with just a few minutes? If your employer offers matching contributions, just obtain the necessary paperwork from your human resources department, complete the donor section, and send it to us along with your gift. If you've already sent in your gift, you can still send us the completed form or online donation information. Please search our matching gifts database to see if your company participates in matching gifts.

Community Health Charities of Illinois
The Les Turner ALS Foundation is a member organization of Community Health Charities of Illinois (CHC), a group of 38 premier non-profit health organizations that provide patient services, public and professional health education, and medical research throughout Illinois. CHC raises funds on behalf of member organizations through employee payroll deduction programs.

Combined Federal Campaign
Contributions from civilian and military federal employees through the Combined Federal Campaign (CFC) are the lifeblood of the Les Turner ALS Foundation. The Foundation's CFC donor code is 5501.
United Way
Check with your employer about how you can direct your United Way contributions to the Les Turner ALS Foundation.
