2017 Celebration of Life

November 12, 2017 @ 1:00 pm - 3:00 pm

Please join us at the Les Turner ALS Foundation’s Celebration of Life to remember our loved ones lost to ALS.

This special service honors all those we have lost to ALS in the last forty years and includes readings by members of the Foundation’s Home and Community Services Team with a candle lighting ceremony and refreshments to follow. This event will take place in the lower level of the Foundation office.

Ways that you can actively participate in the Celebration of Life:

Memorial Plaque Program: With a memorial gift of $175 or more, we would be honored to place your loved one’s name on our annual Remembrance Plaque. Please see this form for more details.
• Include your loved one’s name in the Celebration of Life program book. All names will be read during the service. All names must be received by Friday, October 20th.
• Bring a photo of your loved one for our memories display.

Attendance at the Celebration of Life is not required to participate in any of the above activities.

Space is limited, please RSVP to info@lesturnerals.org or call the office at 847 679 3311 by Friday, October 27
